RESUMO

Objective:To observe the effect of Yi Jin Jing(Sinew-transforming Qigong Exercises)intervention on anxiety in asymptomatic patients with Corona Virus Disease 2019(COVID-19)infection during quarantine.Methods:A total of 160 asymptomatic patients with COVID-19 infection were stratified by gender and divided into an observation group and a control group by the stratified randomization method,with 80 cases in each group.The control group was given basic nursing in the cabin,and the observation group was given additional Yi Jin Jing exercises once a day,20 min each time,and trained continuously until 1 d before leaving the cabin.The Hamilton anxiety scale(HAMA)score was compared between the two groups.Results:A total of 154 cases were included for data analysis in this study,including 74 cases in the observation group and 80 cases in the control group.After intervention,the HAMA scores in both groups increased(P<0.05),while the score in the observation group was lower than that in the control group(P<0.05).In the control group,the HAMA score of females after intervention was higher than that of males.The scores of mental anxiety and somatic anxiety in both groups were higher than those before intervention,while the score of mental anxiety in the observation group was significantly lower than that in the control(P<0.05).However,there was no statistical difference in the somatic score between the two groups(P>0.05).Conclusion:Patients with COVID-19 infection will get anxious during quarantine.Yi Jin Jing exercise can effectively reduce the aggravation of anxiety in asymptomatic patients with COVID-19 infection during concentrated quarantine.

Objective:To investigate the distribution and drug resistance of pathogens causing blood stream infection in patients with pancreatic neoplasms.Methods:Clinical data of patients with pancreatic neoplasms complicated with bloodstream infection with confirmed pathological evidence admitted in the First Affiliated Hospital of Naval Medical University from January 2016 to June 2019 were collected. Bacteria were isolated from blood culture, and microbial sensitivity tests were analyzed by minimum inhibitory concentration and Kirby-Bauer methods. The distribution and drug resistance of pathogens causing blood stream infection in patients with pancreatic neoplasms were analyzed.Results:There were 116 cases (8.5%) with bloodstream infection in 1 372 patients with pancreatic neoplasms. A total of 156 bacterial strains were isolated from blood culture, of which 127 strains (81.4%) were gram negative aerobic bacteria, mainly including Escherichia coli (42 strains), Klebsiella pneumoniae (34 strains), Pseudomonas aeruginosa (12 strains), and 25 strains (16.0%) were gram positive aerobic bacteria, mainly including Enterococcus faecium (11 strains), Enterococcus faecalis (3 strains), Streptococcus angina (3 strains). 4 strains (2.6%) were anaerobic bacteria. The results of antibiotic susceptibility showed that the resistance rate of Escherichia coli to ampicillin was 90.5%, and to cefoperazone-sulbactam was 2.4%. The resistance rate of Klebsiella pneumoniae to piperacillin was 20.6%, and to cefoperazone-sulbactam was 5.9%. The resistance rate of Pseudomonas aeruginosa to imipenem was 41.7%, and no resistant strain was found to cefoperazone-sulbactam. The resistance rate of Enterococcus and Streptococcus pharyngitis to erythromycin were 85.7% and 33.3%, and no strains were resistant to vancomycin.Conclusions:The rate of blood stream infection in patients with pancreatic neoplasms was relatively high. In clinical practice, the distribution of pathogenic bacteria should be understood, the drug resistance should be monitored and antibiotics should be reasonably used, in order to maximally prevent and interfere with the occurrence of blood stream infection.

Objective@#To evaluate a self-designed external fixation restorer used for femoral shaft fractures in children.@*Methods@#From September 2016 to October 2017, 19 children were treated at Department of Pediatric Orthopaedics, The Third Hospital of Hebei Medical University for irreducible femoral shaft fractures using our self-designed external fixation restorer. They were 15 males and 4 females, aged from 4 years and 2 months to 8 years (average, 6.3 years). There were 7 transverse fractures, 11 short oblique fractures and one oblique fracture. The restorer was applied directly to the femur for traction and temporary external fixation. The femoral shaft fractures were reduced closely before internal fixation with elastic stable intramedullary nails. The Flynn criteria for lower limb fracture were used to evaluate the curative effects postoperatively.@*Results@#The operative time ranged from 32 to 45 minutes (37 minutes on average). All the fractures obtained closed reduction. No such intraoperative complications occurred as traction and compression injury to soft tissues like muscle, nerve and blood vessel. Follow-ups ranged from 8 to 20 months (average, 13 months). All the fractures got united after 7 to 15 weeks (average, 10.1 weeks). No implant failure or breakage occurred. The implants were removed 6 to 8 months after operation when the fractures got united. According to the Flynn evaluation criteria at the last follow-up, 18 cases were excellent and one was good.@*Conclusions@#Due to its advantages of simplicity, easy manipulation and direct action on the femur for traction, our self-designed external fixation restorer can improve the closed reduction for femoral shaft fractures in children so that its sustained and effective traction force and high quality of fracture closure avoid surgical opening. The temporarily fixation it provides after fracture reduction can facilitate intraoperative fluoroscopy of the femur.

Objective To probe the X-ray imaging features and clinical manifestations of some rare diseases in male breast.Methods The data of 12 patients with some rare diseases in male breast were collected.The X-ray manifestations and clinical features of the diseases were analyzed.Results The postoperative pathology showed the lipogranuloma in 1 patient,plasma cell mastitis in 2,secondary infection of epidermal cyst in 1,intraductal papilloma in 1,breast cancer in 6 (invasive ductal carcinoma in 4,poor-differentiated adenocarcinoma in 1,and mucinous carcinoma in 1),and breast lymphoma in 1.The digital mammography showed the intramammary solitary masses in 1 1 and multiple ones in 1.10 masses were located at the rear of the areola,and other 2 were opposite.Unclear edge of the lesion was found in 9,spiculation in 1,lobulation in 1,and micro-calcification in 1.Nipple retraction and areola skin thickening in 10 and enlarged axillary lymph node in 1 were also found.The total accurate rate of X-ray diagnosis was 41% (5/12),while that of breast cancer was 83.3%.The total misdiagnosis rate was 59% (7/12),and that of benign lesion was 80% (4/5).Conclusion The diseases in male breast are rare and various,and the X-ray and clinical manifestations are not specific.

Objective:To study the promotion effect of recombinant human acidic fibroblast growth factor ( rh-aFGF) on wound healing in children with surgery or accident injury. Methods: Totally 206 cases of injured children were randomly divided into two groups, the treatment group was given rh-aFGF, and the control group was treated with basic fibroblast growth factor (bFGF). The ob-servation of wound healing, infection and fat liquefaction in the children was recorded in 7 days,and the modification of scar, pigmenta-tion and adverse reactions were observed in 6 months after the treatment. Results:After the 7-day treatment by rh-aFGF and bFGF, the healing rate respectively was 88. 4% and 74. 2%, and the residual wound area was (2. 2 ± 1. 1) cm2 and (3. 9 ± 2. 4) cm2,respec-tively. Six months after the treatment, the scar incidence rate of the treatment group and the control group was 27. 2% and 67. 5%, and the incidence of pigmentation was 30. 1% and 69. 9%, respectively. Conclusion:Rh-aFGF is more effective than bFGF in pro-moting wound healing in children with surgery or accident injury. It can effectively promote wound healing, shorten the healing time, and diminish the area of scar and pigmentation.

Objective Frontal resting EEG asymmetry was used to evaluate the gender effect of emotion regulation.Methods The resting EEG was recorded in 30 healthy volunteers (19-26 years old) by the guide 40 amplifier,and off-line analysis on EEG data were conducted by scan4.3 software.The statistical analysis were performed using SPSS 17.0.Results In normal human,frontal resting EEG alpha power was statistically significant(F=4.918,P =0.035).Asymmetry and gender interaction group (F =0.668,P =0.421),electrode point and gender interaction group (F =0.283,P =0.756),electrode point and asymmetry interaction group (F =1.418,P =0.260),electrode point,asymmetry and gender interaction group (F =1.164,P =0.327) had no statistical significance (P > 0.05).The comparison of male and female asymmetry degree of each electrode point (F4/F3 group t =0.465,F8/F7 groupt=0.809,FP2/FP1 group t=1.542) had no statistical significance (P>0.05).Conclusion Normal human brain hemispheres exist asymmetry.The asymmetry does not exists gender effect.In non-task state,frontal resting EEG alpha power asymmetry can not be recognized as the indicator of emotion regulation ability.

This software provides a convenient and effective self-acupuncture treatment program for the public. It makes TCM acupuncture popular in the public and further pro-motes the development of TCM .

Objective To observe the clinical effects and safety of radiofrequency thermocoagulation in treating the lumbar intervertebral disc protrusion.Methods Thirty patients with lumbar intervertebral disc protrusion,whose diagnoses were confirmed by clinical manifestations and CT findings,were involved in this study.The needle was punctured to the target point of the diseased intervertebral space under C-arm fluoroscopic guidance.After the testing of sensory nerve,motor nerve and temperature was made,the target needles were heated until the nerve radiofrequency temperature meter reached the point of 92℃.This point of temperature was held for 100 seconds and the procedure was repeated for four cycles.Results Six months after the treatment,all patients showed an obvious improvement in VAS,which decreased from 7.83±0.33before operation to 2.37±0.48 after treatment(P<0.05).According to the modified Macnab therapeutic evaluation criteria,excellent result was seen in 9,good result in 12,fair result in 5 and poor result in 4cases,with a total effective rate of 86.7%.No serious complications occurred in all patients.Conclusion Radiofrequency thermocoagulation is an effective and safe method for the treatment of lumbar intervertebral disc protrusion.

Objective To evaluate the effectiveness and safety of interventional thermochemotherapy for VX2 transplanted hepatic tumor model in experimental rabbits.Methods The hepatic tumor model was established in 20 New Zealand rabbits by implanting VX2 tumor cells into the right hepatic lobes of the rabbits.The rabbits were randomly and equally divided into two groups with 10 rabbits in each group:group A (study group)and group B(control group).Via the femoral access,the catheter was placed into the tumorfeeding artery,which was confirmed by DSA.The rabbits in group A received an infusion of 100 ml 5% glucose plus 5-Fu(20 mg/kg)at 60℃ temperature,while the rabbits in group B received an infusion of 100 ml 5% glucose plus 5-Fu(20 mg/kg)at normal room temperature(22-25℃).Before and after the procedure,the tumor size was measured by means of B ultrasonography and the serum ALT was estimated to assess the liver function.The results were statistically compared between two groups.Results After the treatment,the tumor size in group A(study group)became much smaller than that in group B(control group),with P<0.05.And no significant difference in the serum ALT level existed between group A and group B.Conclusion The interventional thermochemotherapy is superior to interventional chemotherapy in inhibiting VX2 transplanted hepatic tumor in experimental rabbits.

Objective To investigate the apoptosis of gastric carcinoma SGC-7901 cell induced by histone deacetylase inhibitor(TSA)and to clarify its mechanisms.Methods The apoptosis-inducing role of TSA on gastric carcinoma SGC-7901 cell was investigated with the help of cell proliferation assay,Annexin V stain,cell flow analyzer and Tunel assay.Western blot,gene chips,real time PCR were employed to study the influence and mechanisms of TSA on the expression of gastric carcinoma cell SGC-7901 p53,bax,etc.Results TSA could induce the apoptosis of gastric carcinoma SGC-7901 cells,increase the expression of p53 and bax,and decrease the expression of bcl-2.survivin and easpase in gastric carcinoma SGC-7901 cells.TSA could transfer AIF and EndoG from mitochondria to nucleus.The apoptosis induced by TSA was brought about through the regulation of multiple apoptosis-related genes,and the apoptosis pathway induced by TSA was caspase-independent.Conclusion TSA can induce caspase-independent apoptosis in gastric carcinoma SGC-7901 cell through the regulation of multiple apoptosis-related genes.

For recent two decades,radiofrequency ablation technology(RFA)has made great progress in the field of the treatment for diseases for its distinguishing characteristic of microtrauma,targeted,effective and almost having no side-effect.At the very beginning,radiofrequency ablation was adopted in treating solid tumors,and since then it has been gradually practiced in treating benign diseases of solid organs such as hypersplenism,prostatic hyperplasia and widely used in almost every system of the body.Here,we review its therapic principle,types and clinical application.

RESUMO

[Objective ] To investigate the improvement of the surgical treatment effect on cerebral palsy with spastic diplegia by performing modified surgeries since 2004. [Method ] Twenty-three cases of severe cerebral palsy with spastic diplegia were operated, 14 male, 9 female; age 3 ~ 12 years; the average age of 5.7 years; their main symptoms were unable to stand and walk independently, with severe hypermyotonia of both lower extremities, and a scissoring type of gait when standing, hip and knee flexing contracture, ankle and foot equinovarus. Physical examination: there was hypermyotonia of the iliopsoas, adductor musculus, hamstrings, triceps surae, anterior and posterior tibial muscle, flexor pollicis longus and flexor digitorum longus. The muscle tone was 3 ~4 grade according to Ashworth criterion. Treatment: The intramuscular tenotomy, fascial division or sliding elongation were performed, to lengthen the tendon of the joint flexion contracture, the lateral half of anterior tibial muscle tendon was transferred laterally to balance the muscle force at the foot. Finally, both lower extremities were fixed by a new designed external frame, to straigthen the articular genu, keep both ankles and feet in neutral position, both lower extremities abducting 30°, and to correct all joint deformities. Family rehabilitation programs started after the external frame was removed 6 weeks later. [Result] All cases were followed up in 1 ~3 years with the average 2. 2 years. The results were evaluated as excellent, good and ineffective, according to correction of the joint deformities, the ability to stand and walk, and parent's satisfaction to the treatment. Of them 21 cases were excellent, 2 cases good, and no ineffective case. [ Conclusion ] For the severe cerebral palsy with spastic diplegia in children, reasonable release of the tight tendons of the multi-joint flexion contracture, balance of muscle force at the foot, correction of all deformities and fixation of the lower extremities with the new external frame, and long term of family rehabilitation after the surgery, are more effective treatment.

Objective To treat femur fracture in neonates using a new homemade harness designed and observe the effec of this harness. Methods Designed a new harness used in femur fracture in neonates,and used this harness in 7(8 femur fracture)csses,including 6 boys(7 femur fracture)and 1 girl,the age from 1 day to 12 days(average 4.7 days).Those cases included proximal thirds(2 cases)and middle thirds(6 cases)of femur fracture.The angle of fracture was from 44°to 83°(average 62.4°)before treatment. Results The angle of fracture was from 0°to 22°(average 14.0°)after treatment using the harness.Hespitalization was from 2 to 3 days.There were no skin sloughing or harness breaking off.All cases were followed up 6 to 36 months(average 21.3 months).All femur fracture healed in good alignment with leg-length discrepancy<1 cm.Movement of hip and knee was normal. Conclusions The harness is a better method to treat femur fracture of the proximal and middle thirds in neonates.Advantages of it include simply operation,minimal hospitalization,minimal cost,and easy nursing.

BACKGROUND: Recently,Illizarov external frame is commonly used to treat adult talipes equinovarus.However,its application is limited in infant and child due to amount of components,large volume and heavy weight. OBJECTIVE: A new three-dimensional external mini-frame was designed to observe its therapentical effects for congenital talipes equinovarus in infants. DESIGN,TIME AND SETTING: Retraspectively analysis of the therapentical effects of the three-dimensional external mini-frame for congenital talipes equinovarus in infants at the Department of Pediatric Orthopedics,Third Hospital of Hebei Medical University between January 2004 and June 2007. PARTICIPANTS: 106 infants (130 feet) with congenital talipes equinovarus,76 males and 30 females aged 1.6 years (range 0.5-3 years),including 80 left feet and 50 right feet,24 cases bilateral feet and 82 cases unilateral feet. Eighty-one cases underwent orthosis or cost treatment but were not cured; 10 cases underwent surgery in other hospitals but all failed,the other cases were not given any treatment. The new three-dimensional external mini-frame was designed by the authors and produced by Hebei Hengshui Kangda. METHODS: According to the age,degree of deformity,being operated or not and pathological changeS of the foot,different treatment methods were applied: Corrcetion and fixation of the foot directly with the new external mini-frame; Limited soft tissue release,followed by corrcetion and fixation of the foot with the mini-frame; Corrcetion and fixation of the foot with the mini-frame after more soft tissue realse on interior posterior foot; Corrcetion and fixation of the foot with the mini-frame after soft tissue release,tendon transfer and restore of muscle force balance. The postoperative external fixation lasted for about 3 mouths. MAIN OUTCOME MEASURES: Evaluatoin of therapentical effects according to Garceau standards. RESULTS: All cases were followed up for 2.5 years (range 0.5-3.5 years). Eighty-six cases (94 feet) were excellent: the foot looked near normal,the calcar pedis was erect,with plantigrade foot in walking,the range of the joint motion in foot was near normal. Seventeen cases (32 feet) were good: the foot deformity improved obviously,but a light adduction was remained in the forefoot,with plantigrade foot in walking,the range of joint motion in foot was good. Three cases (4 feet) were effective: the forefoot adduct lightly,with lightly calcaneus varus,the motion of foot was limitted partly. CONCLUSION: The new 3D external mini-frame shows favorable effects for congenital talipes equinovarus in infants.

[Objective]To explore the effect on spinal growth and devetopment at different month-age of immature rabbits with spinal fusion and fixation;and to explain the mechanism of crankshaft phenomenon after child scoliosis treatment.[Method]Twenty-four 6-weeks-old rabbits and 24 15-weeks-old rabbits were randomly divided into 2 groups.Various lengths of lumbar vertebral region were fused posteriorly and fixed with wires,and the Cobbs angle in the frontal and saglttal planes were measured after the surgery.[Result]The lumbar spine of each group became lordosis with different degree.It was most obvious in the group which were fused in 7 lumbar vertebraes at 6-month-week age.The multiple linear regression outcomes show significance among different month-age,different lengths of fusion region and the Cobbs angle.[Conclusion]In the imature rabbits,if their lumbar vertebrae were posteriorly fixed only may be appeared and getting serious gradually due to the potential growth of anterior spinal column.There has significant correlation among the degree of skelecton immature,fused segment length and spine lordosis.

RESUMO

[Objective]A new type of external frame for the treatment of club foot in three dimensions was reported.The characteristics of this frame and Illizarov frame were compared.[Method]The new frame was consisted of 3 couples with 6 metal rods.The rods have lengthening or compressing force on foot in three dimensions respectively through 4 Kire wires in tibia,calcaneus and tarsals.Sixty-two cases with 85 club feet followed more than 12 months were treated by the frame since Fabruary 2004.[Result]According to Gaeceau standerd and clinical appearance,function of the feet after the treatment,40 feet were excellent,40 feet were good,5 feet were improved,no poor case.[Conclusion]The new type of external frame is safe,effective and reliable for the treatment of club foot in three dimensions.Compared to Illizarov frame,it is more simple,comfortable,cheep and easy to be managed.
